About us

Procare LTC was formed in 2007 by former nationally owned long term care pharmacy leaders that realized the need to go back to the more personalized, customer service fundamentals of quality care.


Procare LTC was founded on a basic premise: to provide the highest level of customer service possible while delivering the best economic value to the facilities and residents we serve. This is achieved through the use of highly qualified and trained staff at every level of our organization. Hammad Shah, RPh

Chief Executive Officer

Hammad Shah, RPh serves as CEO of Procare LTC to set the company vision and growth strategy.

A pharmacist by trade, Hammad joined Procare LTC from SOC Telemed – the leading provider of acute telemedicine technology. As President & CEO, he successfully grew the company’s profitability and EBITDA to expand their network and connect patients to the highest quality clinicians at the right time – regardless of proximity. Hammad also started and previously served as President of FreseniusRx, a specialty pharmacy focused on renal medications to positively impact the lives of patients living with kidney disease.

With over 30 years of experience, Hammad’s track record has proven to influence increased access to care and higher quality of services. He received his bachelor’s degree in pharmacy from Temple University and his MBA from Arizona State University, W. P. Carey School of Business.

Tom Hyde, RPh

Co-Founder and Chief Operating Officer

Tom Hyde, RPh founded Procare LTC in 2007 and served as CEO through June 2020. Now, in his new position as COO, he will focus his role on structure and process, overseeing the administrative and operational functions. With Tom’s experience and expertise in institutional pharmacy, he will provide the foundation needed for Procare’s next chapter. Throughout his professional career he has held positions in consulting, customer service, technology and C-Suite management, arming him with vast knowledge of the industry and its needs.

Tom’s commitment and an undaunted passion towards excellence is evidenced by Procare’s growth under his leadership. He received a bachelor’s degree in pharmacy from St. John’s University.

Paul Meyeroff, RPh

Co-Founder and Growth Leader, LTC

Paul Meyeroff, RPh founded Procare LTC in 2007 and served as President from 2007-2020. In his new role as Growth Leader, he will harness his passion of bringing greater value to residents, owners, operators and clinical staff of healthcare facilities within the long-term care sector. Paul believes value is achieved through a constant eye on the ever-changing reimbursement landscape and proactively identifying customer needs to be at the forefront of care for skilled nursing, assisted living and adult homes.

Paul is an experienced executive with more than 35 years serving within institutional pharmacy organizations. Paul graduated with a bachelor’s degree in pharmacy from St. John’s University and is currently a registered pharmacist in New York and Florida.

Terrye Enslow

Chief Financial Officer

Terrye Enslow serves as chief financial officer for Procare LTC, helping to drive the strategic vision of the company, while also building culture and teams.

Terrye joined Procare LTC from Luck Companies – the largest family owned and operated aggregates materials company in the U.S. There she served as vice president of corporate development, where she led the company’s acquisition, real estate and strategic planning initiatives to grow its footprint and operating revenue. Prior to that, Terrye served in successively senior roles at McKesson Medical, where she oversaw the finance team of the $6B company and was responsible for controllership, financial planning and analysis, strategic planning, accounting operations, real estate and financial systems.

With over 20 years of experience, Terrye’s background is rooted in creating value and impact through clear execution and building high-performing teams. She received a Bachelor of Business Administration degree in accounting from The College of William & Mary in Williamsburg.

Mike Zandri, RPh

Vice President of Business Integration & Account Success

Mike Zandri, RPh brings decades of experience in the institutional pharmacy market to his role as vice president of business integration & account success, as he uses his expertise to ensure a smooth transition for the pharmacies that join Procare. Previously, Mike served as vice president of operations for Procare, supporting the company’s core philosophy of providing best-in-class services to the facilities and residents we serve.

Mike began his career as a pharmacist with Fay’s Drugs — a regional chain of 272 pharmacies across Upstate New York and Pennsylvania – then transitioned to roles in both retail and long-term care pharmacy services for Thrift Drugs and Eckerd. Mike continued his career with NCS Healthcare, later working for Omnicare as a regional director following the acquisition of NCS. Mike earned his degree from Albany College of Pharmacy at Union University.

Sean Brown

Senior Vice President of Information Technology

Sean Brown leads the Procare information technology team, leaning on his experience as a strong technical leader with deep expertise in operations, crisis management, team building and security. Sean is a transformational technology executive noted for driving innovation and growth while reducing costs through enhanced efficiencies.

Before joining Procare, Sean served as vice president of information technology at SOC Telemed, the leading national provider of acute telemedicine solutions for hospitals, health systems, post-acute providers, physician networks and value-based care organizations. Sean was a principal architect of SOC’s hardware solution, which was a key element of the company’s offering to its clients.

Rajiv Dhadwal, RPh

Vice President of Operations

Rajiv Dhadwal, RPh brings nearly 10 years of pharmaceutical industry experience to the role of vice president of operations. Through his deep knowledge and understanding of the inner workings of retail pharmacy, Rajiv is responsible for the operations functions within Procare. Before moving into his current position, Rajiv spent more than five years as Procare’s chief compliance officer, overseeing and managing regulatory compliance to ensure services were always at their highest level and in line with government regulations.

Prior to joining Procare, Rajiv worked for Target and as a pharmacy business development consultant for Woodlands Park Pharmacy in the United Kingdom. He earned his Doctor of Pharmacy degree from the Albany College of Pharmacy at Union University.
